LifeWise Passport™
In addition to providing benefit-rich coverage with a prevention and wellness focus, LifeWise Passport plans allow you to tailor out-of-pocket expenses by providing five options for deductible ($500, $1000, $1500, $2500, or $5000) and three options for office visit copays ($20, $25, or $30).

LifeWise Essentials™ 20
LifeWise Essentials 20 plans are ideal for individuals who want low premiums, coverage for major medical services such as urgent care, emergency and diagnostic services, and who are looking for upfront benefits like preventative care, preventive immunizations and health education.

LifeWise HSA™ 20
The LifeWise HSA 20™ Plan is an HSA-qualified plan with a $3,000 deductible per individual and 20% coinsurance.

LifeWise HSA™
The LifeWise HSA plan has a higher deductible ($5,000 for an individual) but no coinsurance. With the lowest premium of any of LifeWise’s HSA-qualified plans, more savings can be diverted into the HSA.

Preventive Screenings
Preventive screenings are tests your doctor uses to make sure everything’s going well and are covered on all LifeWise plans. Preventive screenings are preventive services under diagnostic and laboratory benefit. Some of the covered screens include:
- Cervical (PAP) cancer screenings
- Prostate (PSA) cancer screening
- Cholesterol Screening
- Bone density screening
Non-preventive Diagnostic and Laboratory Services
LifeWise health plans provide coverage for appropriate diagnostic services for covered conditions that are not included in the preventive screenings list. This means that services such as x-rays, MRIs, EKGs and lab services are covered by LifeWise plans when used to diagnosis covered conditions.
